Lead and heavy metals in toothpaste, what testing found

The short answer

Independent testing has found detectable lead in most toothpastes on the shelf, which sounds alarming and mostly is not. The published risk assessment found the amounts are far below what you get from ordinary food, and the exceedances that did turn up were in children under worst case assumptions, meaning kids swallowing paste rather than spitting. The practical takeaway is not to switch brands in a panic. It is to use less paste on a kid brush and teach them to spit.

This story comes back around every year or two, usually as a headline saying most toothpaste contains lead. The headline is accurate. The conclusion people draw from it usually is not.

A peer reviewed analysis of fifty toothpastes found detectable lead in most of them, with some also containing cadmium, mercury, or arsenic. Only five products came back with nothing detectable.

Then the same researchers did the part the headlines skip, which is calculating whether those amounts actually matter.

What the testing actually found

Detectable does not mean dangerous, and this is where most of the confusion lives. Modern lab equipment can find lead at concentrations that would have been invisible twenty years ago. Lead is in soil, in water, in vegetables, in spices. Finding trace amounts in a product made from mined mineral abrasives is not surprising.

The published risk assessment concluded that the four heavy metals found across those fifty products were not anticipated to increase health risk. Cadmium and mercury were markedly below safety thresholds. Lead and arsenic exposure from brushing was minimal next to what people take in from ordinary food.

Ten of the fifty did exceed a guidance value for lead, and three for arsenic, but only under upper bound assumptions for children. That is the scenario where a kid uses too much paste and swallows most of it, twice a day, every day.

Why the child numbers are the ones worth paying attention to

Adults spit. Small children often do not, and a four year old left to their own devices will squeeze out a stripe of toothpaste the length of the brush because that is what the commercial shows.

That is the actual variable in every one of these exceedance scenarios. Not the brand. The dose.

The standing guidance has not changed and it handles this cleanly. Under three, a smear the size of a grain of rice. Three to six, a pea. Supervise, and teach spitting early. That alone takes any of these products out of the range where the numbers matter.

It is also the same guidance that prevents fluorosis, the faint white mottling on permanent teeth that comes from swallowing too much fluoride during the years those teeth are forming. One habit solves both problems.

If your kid swallows toothpaste, the fix is a smaller amount and a lesson in spitting, not a different tube. Changing brands while the dose stays the same solves nothing.

The part that surprises people

A lot of the products that test highest for heavy metals are the ones marketed as natural, fluoride free, or clay based.

That makes sense once you think about where the metals come from. They are contaminants in mineral ingredients, and clay, calcium carbonate, and charcoal are mined minerals. A synthetic silica abrasive made in a controlled process has fewer opportunities to pick up trace lead than bentonite clay dug out of the ground.

So there is a genuine irony here. Some of the people most worried about what is in their toothpaste have switched to products with more heavy metals in them and less of the one ingredient with decades of evidence behind it.

I am not telling anyone what to put in their mouth. But if the reasoning for switching was safety, the switch may not have gone the direction it was meant to.

Natural does not mean fewer contaminants. Mined mineral ingredients are exactly where trace heavy metals come from.

Some perspective on the exposure

A pea of toothpaste is roughly a quarter of a gram. Even in a product at the high end of what was measured, the amount of lead involved is a rounding error next to a normal day of eating.

Lead exposure in children is a real public health issue and I am not waving it off. But the sources that drive it are old paint, old plumbing, contaminated soil, and certain imported spices and cosmetics, not the quarter gram of paste that mostly goes into the sink.

If you are genuinely concerned about lead exposure for your family, a blood lead test through your pediatrician tells you something. Switching toothpaste brands tells you nothing.

How I would actually choose a toothpaste

Fluoride content first, because that is the ingredient doing the work of preventing cavities.

The ADA Seal of Acceptance second. It is voluntary, so its absence does not condemn a product, but its presence means the manufacturer submitted evidence to an independent panel instead of just running ads.

After that, whatever your family will actually use twice a day. Flavor matters more than people admit, especially with kids.

What I would not do is chase whitening claims or abrasive additives. If you want to know why, we went through the charcoal toothpaste question separately, and abrasion is a bigger practical risk to your enamel than trace lead ever was.

Common questions

Does my toothpaste contain lead?

Probably at a trace level, like most products tested. The published risk assessment found those amounts are not expected to increase health risk, and are small compared to what people take in from ordinary food.

Should I throw out my toothpaste?

No. The exposure from brushing and spitting is negligible. If you have small children, focus on how much paste is on the brush and whether they are spitting it out.

How much toothpaste should a child use?

A smear the size of a grain of rice under age three, and a pea from three to six, with an adult supervising. That handles both the heavy metal question and the risk of fluorosis.

Is natural or fluoride free toothpaste safer?

Not necessarily, and often the opposite for heavy metals. Trace lead and arsenic come from mined mineral ingredients like clay and calcium carbonate, which those products tend to contain more of.

Does the FDA regulate heavy metals in toothpaste?

Toothpaste is regulated as a drug when it contains fluoride, but there is no single specific limit for trace heavy metals in it the way there is for some foods. That gap is why independent testing gets attention.

What actually causes lead exposure in kids?

Old paint and dust in homes built before 1978, old plumbing, contaminated soil, and some imported spices and cosmetics. If you are worried, ask your pediatrician about a blood lead test rather than changing toothpaste.
